COACH CHECKLIST.

Our region 73 expects the coach to have completed certain requirements before picking up the uniforms.
Here is a Checklist the coach must complete and bring on uniform pick-up day.All sheets to be included.

3. Volunteer Contact Information Sheet

4. AYSO Kids Zone Parent Pledge*

* When a parent was not able to attend the coach parent meeting and not signed the Parent Infor Acknowledge Sheet attending the meeting:

The parent is asked to sign the AYSO KIDS ZONE PARENT PLEDGE as soon as possible so the coach can turn it in with the other sheet on uniform pickup day. This form has the same information the coach has presented the parents in the parent meeting.

*) Use this form for team referees other than the adult referees you listed on sheet 3. We will not contact them unless they contact us.
Each TR can only represent two teams max. They can work unlimited games and gift RPs to other teams.

*)) If you have parents willing to help but unsure how they can do this with their time let them complete this form. They can write on the form
what they may feel they can help with, like put-up nets/take-down nets. Give the form to us. We wil contact them and discuss how they can work best to help your team on a schedule that will work for them.
